House Bill 6 Charter Amendment Request Form

House Bill 6 added several requirements that open-enrollment charter schools must include in their charters. This form has been developed to allow schools to submit three of the required amendments collectively. Please complete this form with original signatures and mail it to:

The form must be received no later than December 3, 2001. One form must be submitted for each charter contract (i.e., one for each county/district number).

The governing board of ___________________________________ Charter School, county/district number _________________, submits the following amendments to its charter:

  1. A prohibition against discrimination in the admission policy on the basis of artistic ability as required by Texas Education Code section 12.111(6).
    Choose one:

    __ The charter is amended to include the statement, "Admission will not be based on gender, national origin, ethnicity, religion, disability, academic, artistic, or athletic ability, or the district the child would otherwise attend."

    __ Because the charter already meets this requirement, an amendment is not necessary.
  2. A description of the manner in which the school will distribute to parents information related to the qualifications of each professional employee of the program, including any professional or educational degree held by each employee, a statement of any certification under Subchapter B, Chapter 21 of the Texas Education Code, held by each employee, and any relevant experience of each employee as required by Texas Education Code section 12.111(10).

  3. A statement that all teachers are required to have a high school diploma as required by Texas Education Code section 12.129.
    Choose one:

    __ The charter is amended to include the statement "All teachers are required to have a high school diploma."

    __ Because the charter already meets or exceeds this requirement, an amendment is not necessary.
