Included in this fax are two requests for information and an announcement of a separate mailing from the Division of Special Education. First is a form for all charter schools (open enrollment and 75% At-Risk-Rule) to complete to satisfy information-gathering requirements specified in HB 211. The bill states, in part, that

Each year…in a form prescribed by the State Board of Education,
each…charter school shall file with the board the following information: the name, address, and telephone number of each officer and member of the governing body of the…charter school; and the amount of annual compensation the…charter school pays to each officer and member of the governing body.

Please submit the attached Annual Governance Reporting Form by November 3rd to the Charter Schools Division.
The second request included here is for enrollment information from schools in operation last year and pertains to the federal Charter School Expansion Act. This act provides for additional funds to be distributed to charter schools that have significantly expanded enrollment from one period to the next. For Title I, significant expansion is defined as an increase of 50% or more at a school with a minimum enrollment of 300. For example, if the enrollment of a charter school increases from 250 to 375, the school meets the required criteria.
The period over which this increase must occur is that between the last day of last school year (1999-2000) and November 1, 2000. Please submit the form (entitled Charter School Expansion Act Enrollment Information) included here by November 3rd. If you meet the criteria for receiving additional funds, you will receive a data request form from the Division of Student Support Programs. The next period is from November 1st to February 1st. We will request information from you again in February, and the same process will follow.
The deadlines associated with this additional funding will be strictly enforced.

Third, the Division of Special Education mailed on Monday, October 23, 2000, a data request for a special November Child Count Collection for charter schools only. These forms are due back to the Agency by November 22, 2000 for consideration of additional funds from the Individuals with Disabilities Education Act, Part B for 2000-2001 related to significant expansion of enrollment. Newly opened charter schools will not be asked to respond to this count. While completion of the special November Child Count is not mandatory, only those charters who respond to the data request and meet the guidelines for significant expansion outlined in the instructions will be considered for additional IDEA-B funds.

Note: If the governing board of the sponsoring entity has delegated authority to a charter school board, the information required below must be provided for members of both boards. Officers of the charter school include the chief executive officer of the sponsoring entity in addition to the chief executive and other officers of the charter school.
