must provide copies of:
a) Proof of residency
b) Child's social security card
c) Original birth certificate
d) Immunizations record
If
you need to make copies, please go to the copy station.
2.
Go
to the nurse’s station for immunization verification.
3.
Complete
all enclosed forms.
4.
Go
to final registration station to have all documents checked: proof of
residency, birth certificate, and social security card.
Please
make sure all your forms are completed in full.
We will be unable to process your child’s registration if all forms are
not complete.
